A customized care plan is developed to meet each patient’s individual needs. Our physicians are involved throughout the treatment process, working with the therapy team, to ensure progress is achieved and goals are realized. If additional services are required, our team will coordinate care with other specialty physicians. Often the emotional implications of dealing with injury and pain can create barriers that can impede optimal recovery. Your First Appointment
What to Expect
It is important to complete all of your paperwork prior to your appointment at iSpine Clinics. Upon scheduling you will receive electronic paperwork to complete and return to us so that the information can be reviewed in advance by a physician. If for some reason you were unable to complete your paperwork prior to your appointment, you must show up 30 minutes early.
Be Prepared for Your First Appointment by:
- Ensuring your paperwork is completed and returned prior to your appointment
- If not, please arrive 30 minutes prior to your appointment time
- Bring a valid picture ID
- Insurance card
- A Complete Medication list
- Calendar to set up future appointments, if needed

We know that living with muscle, bone or joint pain is no fun. We will do our best to schedule you with one of our specialists as soon as possible. We pride ourselves on our ability to accommodate your need for a specialized medical evaluation as quickly as possible.
When calling to schedule your first appointment, please have your insurance information available. We will also want to know if your pain or injury is the result of a car accident or workplace injury. Any information you can provide us regarding claim numbers, adjustor names, attorney names, etc., will expedite our ability to accommodate you in a prompt fashion.
If you have had prior consultations with other providers, imaging studies, procedures or surgeries, it is helpful to obtain those records prior to your appointment. Once you schedule with us, you will receive electronic paperwork which will include a Release of Information (ROI) form. Please complete it and return it to us as soon as possible so we can gather your medical records prior to your appointment. This will allow us to make the most comprehensive diagnosis and care plan for you.
